Intra Cytoplasmic Sperm Injection

Intracytoplasmic Sperm Injection (ICSI) has revolutionized the field of assisted reproductive technology (ART) and offers hope to couples struggling with male infertility issues. This advanced fertility treatment involves the direct injection of a single sperm into an egg, bypassing natural barriers to fertilization. Understanding the procedure and its indications is crucial for couples considering fertility treatments.

Dr Poonam Goyal is a pioneer in the field of Assisted Reproductive Technologies(ART). With over 14 years of experience in the field of ART, She provides hope to couples struggling with Infertility. With her expertise in various types of Assisted Reproductive Technologies including IVF, ICSI, Male infertility, Surrogacy, fertility preservation and many more, she has helped thousands of couples overcome ailments related to infertility.

What is ICSI?

An advanced method of in vitro fertilization (IVF) called intracytoplasmic sperm injection (ICSI) was created to address male infertility issues such as low sperm count, low sperm motility, or abnormal sperm morphology. Unlike traditional IVF, where eggs and sperm are combined in a dish and left to fertilize naturally, ICSI involves the precise injection of a single sperm directly into an egg using a microscopic needle.

When is ICSI Needed?

ICSI may be recommended in cases where natural fertilisation is unlikely to occur due to male infertility factors, including:

  • Low Sperm Count: When the sperm count is too low to achieve fertilization through conventional IVF.
  • Poor Sperm Motility: When sperm are unable to swim effectively to reach and fertilise the egg.
  • Abnormal Sperm Morphology: When sperm have structural abnormalities that hinder their ability to penetrate the egg.
  • Previous IVF Failure: When couples have experienced failed fertilization or low fertilization rates with conventional IVF.

ICSI Procedure

The ICSI procedure typically follows the same steps as traditional IVF, including ovarian stimulation, egg retrieval, sperm collection, fertilization, embryo culture, and embryo transfer. However, the key difference lies in the fertilization step, where individual sperm are selected and injected directly into mature eggs under a high-powered microscope.

  • Lifestyle Modifications: Ovarian Stimulation: The woman undergoes ovarian stimulation with fertility medications to stimulate the ovaries to produce multiple eggs.
  • Egg Retrieval: Mature eggs are retrieved from the woman's ovaries using a minimally invasive procedure called transvaginal ultrasound-guided follicle aspiration.
  • Sperm Collection: Sperm are collected from the male partner through ejaculation or surgical sperm retrieval techniques such as testicular sperm extraction (TESE) or epididymal sperm aspiration (TESA).
  • Sperm Selection and Injection: A highly skilled embryologist selects the best-quality sperm and immobilizes it using a specialized holding pipette. A tiny needle is then used to inject the sperm directly into the cytoplasm of the egg.
  • Fertilization and Embryo Culture: After injection, the fertilized eggs (embryos) are cultured in a laboratory for several days to allow for embryo development.
  • Embryo Transfer: The most viable embryos are selected for transfer into the woman's uterus, typically on day 3 or day 5 after egg retrieval.
